First Time Visit
If you are visiting us for the first time, please arrive 15 minutes before your scheduled appointment to allow time for registration. Please bring your medical insurance card(s), a photo ID and a list of any medications you are currently taking.
Emergency and After Hour Calls
If an emergency arises after hours or on a weekend, your call will be directed by our answering service. In severe situations or ones in which you are in doubt, go directly to the emergency room at your nearest hospital.
Insurance and Fees
We accept many insurance plans. Prior to scheduling your appointment, please verify with your insurance carrier, that your plan is accepted here. Please bring your current insurance card with you to each visit.
We will file a claim to your primary insurance carried and secondary carrier, if applicable, when the information has been provided. You should notify the office immediately regarding any changes in your insurance carrier or plan.
Please note, Dr. Neri does not participate in Managed Care plans, Workman’s Comp or Personal Injury.
Prescriptions
Prescription renewal should be taken care of at the time of office visits. If you need a refill please call your pharmacy directly, This is a service they provide for you.
If you require a written prescription, please call 630-654-3636. We request 24 business hours notice for refills. Medication refills are handled during business hours only.
